No Show Policy & Procedure for Office Visits and Procedures (08/10/2022)

1. Cancellation/No show policy for office visits.


We understand that there are times when you must miss an appointment due to weather, emergencies or obligations for work or family. However, when you do not call to cancel an appointment, you may be preventing another patient from getting much needed care.


** For established patients, we kindly ask at least 24 hours notice for all cancelled appointments. If an appointment is not cancelled or if an appointment is missed you may be charged a twenty-five dollar missed appointment fee. This fee will not be covered by your insurance and will need to be paid in full prior to scheduling additional appointments.


** For new patients, if an appointment is not cancelled at least 24 hours in advance, you may be charged a fifty-dollar new patient fee. This fee will not be covered by your insurance and will need to be paid in full prior to additional appointments.


** ANY patients with 3 or more missed appointments, repeated last minute cancellations, or chronic lateness, may be discharged from our practice. In that occurrence, our providers will only be available for emergent care for thirty days and a new medical provider will need to be established by the patient.


2. Cancellation/No show policy for outpatient procedures.


Due to the large block of time needed for your procedure, last minute cancellations can prevent other patients from receiving care and cause added expenses to the office. If procedures are not cancelled at least three days in advance, you will be charged a seventy five dollar fee; this is not covered by your insurance and will need to be paid prior to scheduling any additional procedures.
